Enabling Long-Term Success Through Comprehensive Transition Support
Aftercare begins with a thoughtfully developed plan. From day one in treatment, you and your teen will begin working on what success will look like at home. That plan then provides the framework for accountability for your teen. While your teen is in treatment, a dedicated case manager will help you and your teen tailor an aftercare plan that fits your family’s needs. Then, when your teen comes home, that same case manager will continue to check in, follow up and provide support.

Up to 6 Months of Additional Support Helps Each Teen Transfer What They Have Learned in Treatment to the Real World.

Establishing Peer Support
Positive peer support creates an environment where growth, accountability, and healing can thrive. Being part of a peer group who “gets it” can reduce loneliness and increase hope. It reminds individuals they’re not alone in their struggles.
Additionally, peers can hold each other to higher standards in a way that feels supportive, not punitive. Teens are especially responsive to influence from their peers, so when that influence is healthy, it reinforces positive behavior and progress.

Finding a Mentor
A mentor provides guidance, wisdom, and perspective that can accelerate healing and development in ways peers or professionals alone may not. Mentors have often walked a similar path. They’ve faced challenges, made mistakes, and learned from them. Their experience can help someone avoid pitfalls and make wiser choices earlier in the journey.

Continuing Therapy
Continuing therapy in recovery is crucial because healing is not a one-time event—it’s a journey. Therapy provides the tools, structure, and support needed to sustain and deepen that healing.
Recovery isn’t linear. Continued therapy helps individuals recognize warning signs, navigate triggers, and stay on track—especially during life transitions or times of stress.
